Manchester City have one hand on the Premier League trophy, and they will look to claim the title for sure as they host West Ham on the final matchday of the EPL season.

Sitting two points adrift at the top of the table, victory at the Etihad would confirm an unprecedented fourth straight top flight championship, which has never been done before in Premier League history. Yet a draw may not be enough if Arsenal defeat Everton at the Emirates Stadium.

No team has come into the final matchday with the lead atop the table and failed to win the title since the Premier League began play in 1992.

West Ham, meanwhile, sit ninth in the Premier League and have nothing competitively to play for, but would be motivated here to seek what would be a historic and memorable result if they were to scuttle Man City's title chase on the precipice of history.
